This is an additional benefit that provides coverage on Total and Permanent Disability due to bodily injury or disease.

• All medical records related to the claim (Refer to Other Additional Requirements)
• Caregiving questionnaire (to be accomplished by a certified caregiver or private duty nurse)
• Complete Neurological Exam
• Certificate of Employment - for employed claimant including the following:
  • Hire Date
  • Termination date and reason for termination
  • Attendance record
