Here's a convenient way to file a claim. Simply scan your documents and submit the requirements online wherever you are.

Claims requirements

We're sorry to hear that you've been hurt and we're here to help you focus on your recovery. To get started, please prepare clear copies of the requirements below:

1. Valid Government ID
2. Attending Physician's Statement (APS)
3. Police or Narration Report
4. Official Receipts (OR)

1. Valid Government ID
2. Attending Physician's Statement (APS)
3. Hospital Statement of Account (SOA)

We would like to extend our deepest sympathies for your loss. Nothing can be harder than losing someone close to us, that's why we're here to help you in this trying time. To get started, please prepare clear copies of the requirements below:

1. For the Insured:
  • Death Certificate of the Deceased
  • Police Investigation Report (If the cause of death is due to an accident)
2. For the Beneficiary
  • Valid Government ID
  • Marriage Contract (If the Beneficiary is the spouse)
  • Birth Certificate (If the Beneficiary is below 18 years old)
